By my signature and photocopy of my identification, I hereby grant permission to the person named and identified to request and/or present my UT Arlington Education Documents (Diploma(s) and/or Official Transcript(s) to the Notary Public of the UT Arlington Registrar’s Office on behalf for purposes of Authentication.

Additional Requirements *
By submitting this form, I acknowledge that additional documentation will be required before your notarization request can be processed. The notarization process does not start until all required documentation is received. A staff member will email you with any needed forms and/or appointment times.
