Request Permission for Elective Lessons

This form is for current UTA undergraduate and graduate students to request permission to enroll in elective lessons.
Undergrad: MUSI 0171 (1/2-hour) or 0271 (1-hour)
Graduate: MUSI 5171 (1/2-hour) or 5271 (1-hour)

Before you begin this form, make sure that you have a saved screenshot or PDF of your email correspondence with your requested professor where they confirm availability and permission. The screenshot or PDF must clearly show the instructor's name, your name, and the date.

The okay from the professor DOES NOT guarantee elective lessons enrollment. There are many factors that are taken into account before final permission to enroll is granted by the department. You will be contacted by email to inform you if you have been given permission to enroll.

Please be aware - elective lessons have a one-time fee on top of regular per credit hour tuition cost.
A 1/2 hour lesson is one credit hour, and incurs a $350 additional fee. A 1 hour lesson is two credit hours, and incurs a $700 additional fee.
